Malignant hyperthermia
Symptoms...
- Inability to relax after Suxamethonium.
- Increased muscle rigidity.
- Tachycardia.
- Increased end-tidal CO2.
- Cyanosis.
- Patient becomes hot and flushed.
- Temperature rises at several degrees per hour.
- Increased oxygen consumption.
- Respiratory and metabolic acidosis produce a drop in pH.
- Rise in serum Potassium.
Treatment...
- Discontinue the troublesome agent.
- Turn off volatile agents.
- Hyperventilate the patient with high flows of oxygen.
- Use a new non-rebreathing circuit.
- Give cool infusion of saline - not Hartmann's.
- Give Dantrolene: 2-3 mg/kg, then 1 mg/kg.
- Topically cool with ice, fluids, cooling blanket.
